Detail Basketball AAU

Out-of-state travel consent. I hereby grant permission for my child to travel with Detail Basketball AAU coaches and staff to Dallas, TX for the duration of the scheduled trip. I understand that the coaching staff will serve as the supervising adults during all team activities and travel. I consent to my child traveling out of state with Detail Basketball AAU staff.*

Emergency medical authorization. In the event of a medical emergency during travel, and if a parent or guardian cannot be reached immediately, I authorize Detail Basketball AAU coaches and staff to consent to emergency medical treatment on my child's behalf, including transportation to the nearest appropriate medical facility.

I authorize emergency medical treatment on my child's behalf if I cannot be reached.*

Supervision acknowledgment. I understand that Detail Basketball AAU is responsible for supervision only during scheduled team activities. I accept responsibility for arranging my child's transportation to and from departure and arrival locations unless otherwise stated by the organization.

Code of conduct. I understand that all conduct expectations outlined in the 2026 Parent and Player Agreements remain in full effect during travel. Violations may result in the player being sent home at the family's expense.

I understand that program conduct standards apply for the entire trip.*
